    Substance use and mental health disorders are rarely isolated. They’re often compounded by poverty, housing instability, unemployment, untreated medical conditions, and involvement with the criminal justice system. Without support, these challenges can lead to relapse or re-incarceration.

    Case Management provides the structure and advocacy individuals need to navigate life’s complexities while working toward sustained recovery.

    Our case managers develop individualized recovery plans in collaboration with each client. These plans are designed to promote autonomy while ensuring clients stay connected to services and supports that meet their evolving needs.
